Veterans Administration

1. Two VA Research & Development forms are required:

   • VA R&D Application

   • VA Form 10-1436 "Research and Development Information System Project Data Sheet"

2. Be certain to provide all supporting documents and copies as requested on the application.

3. Questions can be directed to: (317)554-0000 ext. 2526.

4. Completed packets should be delivered or sent to: VA Medical Center, Research and Development Committee, D3041.
