Institutional Review Board (IRB)

1. All required forms can be found on the IRB website.

2. The following is required for Full IRB Review:

   • Documentation of Review and Approval

   • Summary Safeguard Statement

   • Informed Consent Statement (sample template)

3. The IRB frequently updates the packet; download current IRB packets and copy forms as needed.

4. Follow the instructions found in the packet and on each form requested.

5. Be certain to provide the documents and required copies as requested.

7. Completed packets should be delivered or sent to: Research and Sponsored Programs, IUPUI campus, UN618.
